Preparing for Your NDIS Plan Reassessment: A Practical Checklist

Information current as at 4 November 2026

If your support needs have changed over time, a plan reassessment may be one way to ask the NDIA for a new plan. Preparing clear, recent information can help you explain what has changed and what support you need. Every person’s circumstances are different, and a request does not guarantee a particular outcome.

First, check what kind of plan change you need

The NDIA describes a plan reassessment as a new plan with different supports, usually considered when there has been a significant change in your ongoing support needs. Urgent, minor or short-term changes may fit a plan variation instead. If you are unsure, ask your my NDIS contact which process applies to your situation.

Since 27 August 2026, a reassessment request must be made by the participant, their plan nominee or a child representative. A provider can help you organise information, but cannot request the reassessment on your behalf.

A practical preparation checklist

Before you complete the request form, it may help to gather notes about:

  • What has changed in your daily life, living arrangements, health or the help available from other people.

  • How the change affects the tasks, activities or goals that matter to you.

  • Which supports are working well, and where your current plan no longer meets your needs.

  • What support you are asking for and how it relates to your disability support needs and goals.

  • Any community or mainstream supports you use alongside NDIS supports.

Gather recent information

The NDIA says evidence should explain the significant change to your ongoing support needs. This may include relevant assessments or reports from treating health professionals, therapists or support workers. Its guidance says evidence will usually be recent—generally within four months of the request and after the current plan was approved. Check the current NDIA instructions for your circumstances.

Ask the person preparing a report to describe your needs in practical terms: what you can do now, what has changed, what support helps, and what may happen if that support is not available. Share your goals and preferences so the information reflects what matters to you.

Complete the form and keep a copy

Use the current plan reassessment request form and include the evidence the NDIA asks for. Keep a copy of what you submit and note the date. You can keep using your current plan while the NDIA decides whether to reassess it. If the decision does not meet your needs, the NDIA explains review options on its website.

For the latest requirements, read the NDIA’s guide to plan reassessments and its update on changes to who can request a reassessment.
